INFORMED CONSENT FOR LIFE COACHING

General Information-  Life coaching is defined by the American Psychological Association  as “a form of teaching and encouragement designed to help people  identify and meet goals of importance to them in their personal and work lives”. Coaching is a professional relationship in which personal goals and information are shared as well as a contractual agreement. Given as such it is important for us to come to a clear understanding of our work together and what rights and responsibilities, we each have. The life coaching process is unique in that we will focus on clear expectations and positive goals. Through positive motivation, honesty, realistic goal setting and dedication we can work towards achieving your goals.

The outcome of your coaching process depends largely on your willingness to engage in this process, which may, at times, result in challenges or even considerable discomfort as you step outside of your normal routine and comfort zone to reach agreed upon goals. There are many outcomes that can be procured from the coach-client relationship. You, the client, are responsible for your own mental, physical, spiritual and emotional well-being and will not hold the coach responsible or liable for any actions or inactions that arise from any services provided by the coach and release Rock Your Family from any liabilities. You, the client, will hold full responsibility for the effort and outcomes of the coaching sessions.

You, the client, understand that life coaching is NOT therapy or counseling and under no circumstance will the life coach try to treat, diagnose, or prevent any mental health disorder. You understand that the services offered are a partnership in which you will be in control of your specific goals and outcomes.

You have the right to terminate or discontinue services at any time, for any reason.

You agree to be honest, to give and receive feedback to help enhance the client/coach relationship, and in return the coach will do the same.

You understand that coaching includes many different areas of your life, including but not limited to work, finances, health, relationships, and education. You agree that it is your responsibility to regulate how to handle these issues, how you want to incorporate coaching principles into those areas and how you apply choices to reach your desired outcome.  

PRIVACY AND CONFIDENTIALITY

All information is assumed private and will only be discussed with the client unless specific release of information is given in writing. You have the right to expect all information is kept confidential between the Coach and you as the client. The only information that may be shared is under any state governed mandatory reporting. During coaching we should rarely come across a mandatory reporting situation, since the focus will be on positive motivation for reaching agreed upon goals. However, should any type of child abuse, neglect, suicidal threats, or threats of harm to someone else become known, then mandatory reporting would apply.

Sessions will be conducted over a platform that could include, video chat, email, text, phone or face to face sessions.

In couples coaching there is an automatic release of information between all involved parties and staff, meaning that if any particular party were to disclose information to any staff that information is not individually private but rather is information between all participating parties and staff and may be used at the discretion of the coach and/or coach’s supervisor to obtain necessary or proper recommendation on  goals.

Occasionally your coach may need to consult with other professionals in their areas of expertise in order to provide the best outcome for you. Information about you may be shared in this context without using identifying information, except in the context of referrals.

If we see each other accidentally outside of the office, your coach will not acknowledge you first. Your right to privacy and confidentiality is of the utmost importance to us, and we do not wish to jeopardize your privacy. However, if you acknowledge your coach  first, he/she will be more than happy to speak briefly with you, but feel it appropriate not to engage in any lengthy discussions in public.

In selecting or in working with a particular team member, you acknowledge that you have read and understand that team member’s profile and biography and that you elect in and of your own accord to work with that team member. You agree to their coaching style, credentials, and rate. You agree to self-pay or paying out-of-pocket for these services.
